HPI: R.B. is a 27-year-old Caucasian male client who visited a psychiatric clinic complaining of

persistent mood swings, auditory hallucinations, and visual hallucinations for the past one year.

R.B. reports that the condition started after he divorced his wife a year ago, who informed him

that she would kill him one day. This statement from his wife has caused significant distress to

R.B., leading to mood swings and hallucinations. The mood swings alternate as a week of good

mood and another week of depressed mood. However, the hallucinations occur during the

depressed mood period. In this case, during the good mood period, the patient experiences

increased risk-taking, elevated mood, unwillingness to sleep, increased energy, talkativeness, and

racing thoughts. However, when he has a depressed mood, the client experiences low mood,

increased need for sleep, reduced energy, social withdrawal, difficulty concentrating, and

worthless. During this period, he also sees people coming with swords ready to kill him, hears

some voices telling him that his divorced wife will kill him tonight, and believes that his food is

poisoned whenever he cooks. This has reduced his appetite, and he only eats food that does not

require cooking, such as fruits. R.B. usually develops a depressed mood and hallucinations when

he remembers their good moments with his wife. On the other hand, the good mood period

develops when he remembers he is still young to make a significant change in his life. Whenever

the moods occur, the client experiences them both during the day and at night. He considers the

depressed mood being so severe that it disrupts his normal functioning. However, the client
