Thank you for sharing your interventional approach in this cease. I particularly find the need for emphasizing on healthy living for this patient. Discussing the dangers of smoking and the need to stop smoking ensures that the patient reduces the risk of other diseases and even aggravating the existing condition. The choice of withdrawing remedies with severe side effects and consideration of alternative medication with minimal side effects helps in patient compliance and achieving best patient outcome (Zuckerman et al., 2018). The weight gain resulting from the use of mirtazapine could lead to more severe conditions linked to weight gain. This informs the need for withdrawal as well as the benefit that could be offered by an alternative medicine with fewer side effects (Zuckerman et al., 2018).

Buspirone comes as an alternative with fewer side effects compared to Cymbalta. Buspirone is efficacious n treating anxiety and patients who present with co-exciting depressive symptoms (Shin et al., 2020). The goal of managing symptoms and reducing side effects of the drugs may be achieved through the choice of medicine that offers effective therapeutic effect and minimal side effects (Shin et al., 2020). Efficient treatment requires a discussion on the treatment approach and follow-up plan that entails details on:

• The need for antidepressant to handle the symptoms
• The side effects of the chosen medicine on the adolescent patient
• Highlight the available alternative of medicine for the treatment of the disease
• Offer parental guide to the parent to ensure best utilization of the prescribed medication
• Discuss and agree on a weekly follow up plan
• Discuss on the possible symptoms to look out and the need to seek immediate attention in case of any severe side effects.
