The implementation stage of the evidence-based practice (EBP) research study might be the most complex and difficult. (Melnyk & Fineout-Overholt, 2019). Several elements, such as the variety of stakeholders, the organizational environment, the available resources, timeliness, sustainability, and adoption, have an impact on how difficult it is to achieve practice change. (Melnyk & Fineout-Overholt, 2019). An organized plan can be key to a project's success. The implementation strategy for the EBP project proposal on diabetic self-management education is discussed in the following paragraphs. The setting, timeframe, budget, and resources, along with the research design and evaluation methods, will be discussed. In addition, measures for overcoming potential obstacles will be identified. Setting and Access to Study Participants The setting for this evidence-based practice (EBP) project is an inpatient unit where patients are admitted with diabetic-related complications. Inclusion criteria include adult diabetes patients whose Hba1c level was tested upon admission. The data that will be supporting this implementation will be derived from the patients with type II diabetes from the hospital EMR system (EPIC). Patients’ A1c levels would be compared before and after the education, The patients would receive project-related education. We will follow up with patients through post- discharge calls. The Project Manager will train the employees directly involved in self- management education. The staff would be given educational handouts and a plan would be developed for scheduling patient education sessions. Patients undergoing the initial implementation of this EBP will be required to sign an informed consent form.Informed consent involves the client actively and autonomously choosing to be a part of the project and giving permission to disclose their data for the project and endorsing and authorizing the project plan
