Week 1 Discussion: Building a Health History

Introduction

The development of an accurate health history of a patient is fundamental for the initiation of the right plan of care for patients and hence a nurse is supposed to have the skills for such a process. The process will allow one to get to know of the patient, the past concerns of the patient, the issues that the patient has had from the past, issues of gender, ethnicity, and the environment. One of the factors that is fundamental for effective collection of this data is the aspect of communication. In this discussion, the purpose is to discuss the aspect of communication and building a health history for a 14 year old biracial male who is living with his grandmother.

Summary of the interview and a description of the communication techniques

The preferred method that will be used in the communication with the patient will entail the use of target questions for building up on the health history.
