The process of diagnosing a patient is carried out upon completion of the collection of data. The data that is collected on the history of the patient is vital because it will allow for the determination of the diagnosis and reduce the differential diagnosis of the patient. In the process of collection of the health history of the patient, there is a need or consideration of various categories of the health history of the patient. This is because there are varying risk factors associated with different health measures and standards. As much as there is an ease in the process of collection of data one is supposed to ensure that there is an assurance of the basis of determination of the diagnosis of a patient based on the data collected. In this paper, some questions are to be used in the process of collection of the health history of the patient, and the second part of the paper, there is a reflection on the possible challenges that may be experienced in asking the questions on health history and the experience that one gain with the implementation of the script.

Health History

The process of collection of health history focuses on the determination of the past health information of the patient, collection of the information on the physical examination of the patient, and putting the information in the record for use in deciding on the diagnosis of the patient (Wang et al., 2018). The data that is collected in this case is the personal information of the patient and the preliminary data on their health. The patient is supposed to be assured of their privacy and they are supposed to be comforted to ensure that they will share the right information.

In the process of collection of the health history of the patient, there should be an assurance of confidentiality in the data that is shared. The patient is supposed to be assured that their information will not be shared with unauthorized persons. Ginsburg, Wu, and Orlando (2019) allude that as a healthcare provider, one has an obligation of protecting the data that is provided by the patient at all times, and hence they must assure the patients of the protection.

The script will help in the provision of information that is supposed to be saved in a well-protected area and that the database is supposed to be immune to any attacks from unauthorized personnel. The assurance of confidentiality in their data will ensure that the patient can be able to be free in sharing their information.
