Subjective:
CC (chief complaint): ” She thought you were going to help me be better” Child’s mother HPI: Dev Cordoba is a 7-year-old boy who is brought by her mother to the clinic to get him to be better due to the challenges of fear and anxiety that he is facing. The report provided show that Dev has had issues of fear of being separated from her mother since the time that his father went and did not come back. At the moment he is having trouble being able to go to school and although he may be attentive he ends up looking at the window with the fear that her mother may not come for him after school. He faces other issues when at school including the issue of being bullied by others to the extent that he threw books to one of them called Billy. He has the fear that his mother loves his little brother more than he loves him and this worries him. He claims that he intends to get help from the clinic to be able to manage his anxiety condition when he is not with his mother. There has not been a report of the child has had any issues of use of any medication and no reports of having any known allergies. He has never been hospitalized and there is no report of having been exposed to any medical issues. He is not using any medication even for his condition as per the mother….
