S.T. is a 60-year-old Caucasian male who came today for his three-month follow-up on his hypertension and Diabetes Type 2. Patients was diagnosed with DM Type 2 four years ago. His last visit was three months ago and states his health has been good and his glycemic control has been good, with blood sugar averaging between 120s-130s. He is taking metformin glimepiride and Levemir insulin. Levemir was increased to 40 units BID during the last visit due to A1C being 9.1. He denies hypoglycemic events. Denies polyphagia but does state that he has increased thirst and has to "pee more than normal." He is not exercising and eats fast food at least twice a day. He states he has had blurry vision in the afternoons that began "about a week ago." He denies having unexplained weight loss. He denies mood changes or irritability, fatigue/weakness or extreme hunger.
