Large group are assigned to the patient who are fighting prolonged diseases or having major illnesses like high blood sugar. Therapy supply chain management refers to how doctors work together to meet their patients’ needs by providing the right treatment at the right time and in the right place. According to the majority of healthcare professionals, treatment teamwork is critical for a successful outcome. When patients see multiple clinicians for chronic diseases, improving the standards decreases the risk of misdiagnosis.
Continuum of care is not a natural occurrence. According the AHRQ (Agency for Healthcare Research and Quality) healthcare communication is defined as “deliberate efforts to promote the accessibility of efficient affordable care.” EHR system, collaboration, healthcare management, are all crucial parts of this service (Jones., et al 2019).
