Objectives:

 

1.      To gain knowledge on ethical and policy guidelines and issues in elderly care. The goal entails gaining a more profound understanding of ethical issues surrounding elderly care and policies governing elderly care. The knowledge will be essential in avoiding and solving ethical dilemmas in patient care.

  1. To drive positive changes in geriatric care. The goal entails assessing gaps in my healthcare facility and designing change projects to improve the care of the senior population.

3.      To Enhance my communication skills. The goal will ensure I can converse with colleagues and team members in a multidisciplinary care team. Communication skills will also be essential in communicating with elderly patients during nurse-patient interaction. The skills will also improve my confidence when meeting with doctors and new patients during my practice.

4.      To strengthen organizational skills. These include effective management of time and resources. The skill will be essential in managing patient volume in a congested work environment, providing quality care in settings with resource constrain, and maintaining accurate patient data records for future reference.

5.      To increase medical knowledge. The goal entails increasing my clinical knowledge in diagnosing and managing illnesses affecting the elderly population. It also entails improving my knowledge in differentiating body changes related to aging and illnesses. The knowledge is essential in providing quality care to the elderly population and addressing complex cases.

6.      To develop leadership skills. These are skills used to organize others towards a common goal. These skills will be essential in coordinating activities in a multidisciplinary care team.

 

Resources and Strategies:

 

1.     The strategies I plan to utilize include consultation with my preceptor and the more experience colleagues, attending lectures, conferences, and workshops, and participating more in administrative tasks. The resources I plan to use include preceptors, more experienced healthcare practitioners, facility administrators, lectures, conferences, and meetings.

2.     The strategies I will use to identify the gaps in the facility and the senior population include conducting population data analysis and assessing the organizational systems. The resources I will use as sources of information include colleagues, patients, and patients’ data.

3.     The strategies I plan to use to improve communication skills include constantly modeling a good relationship with other nurses, constantly seeking advice and feedback on my progress from the preceptor, seeking shared past experiences from other nurses, and observing how other clinicians communicate with patients and colleagues. The resources I plan to use include online communication programs, preceptors, and more experienced practitioners.

4.     I plan to improve organizational skills by progressively taking an increased patient load until I can carry the whole load, preparing worksheets and reports after every shift, documenting daily tasks, and taking time management online courses and exercises.
