Past Psychiatric History:

The patient reports recent inpatient psychiatric admission (05/26/2023). Prior to that, the patient

had never been inpatient. Patient will be receiving outpatient services. Patient reports a prior

history of suicide attempt, and endorsed suicidal ideation which lead him to IP psychiatric

hospitalization.

Family History:

Patient denies prior family psychiatric or medical history. Patient denies a family history of

substance/alcohol abuse /use.

Medical History:

Current Medications:

oGabapentin, Wellbutrin, and Naltroxene

Allergies: PCN and Amoxicillin

ROS:

GENERAL: No weight loss, fever, chills, weakness, fatigue

HEENT: denies visual loss, blurry vision, no hearing loss, no sneezing, no congestion

SKIN: denies rash or itching
