Objective 2: By the end of this practicum experience, I will have gained a confidence and will be able to utilize the technology system within my facility when providing patient care. Planned Activities: When reviewing patient’s medical information and documentation for plan and proposed treatment, I hope to utilize the technology charting system that my facility uses. Of course, I will have my document through Meditrek to learn as well, but doing so within the facility will prepare me for post practicum and my preceptor will evaluate whether my documentation is correct and suggest areas for improvement. Mode of Assessment: Verification will be documented in Meditrek PRAC Course Outcome(s) Addressed:  Another skill that I hope to learn throughout this practicum experience is the depression screening. I hope to perform this assessment on the patient’s that will benefit the most for proper treatment. In addition, I hope to utilize this skill in collaboration with enhancing my technology weakness by documenting the depression screening tool as appropriate by the facility. I will turn to my preceptor if I am in need of assistance when reaching this goal and completing this skill. Objective 3: By the end of this practicum experience, I hope to advance my adult medical knowledge when it relates to disease processes, treatments, diagnosing, etc. and in turn, I will have gained a confidence that will benefit my future advanced practice nursing career. Planned Activities: Since this is my first rotation, I am hoping to observe initially the way my preceptor provides care. This include observing the way they assess, plan, diagnosis and treat. By week 5, if not sooner than week 5, I am hoping to be able to be more independent and from learning from my preceptor I hope to be able to assess, plan and diagnosis myself. I am hoping this will enhance my clinical knowledge and provide me with the information I may lack as a provider. I hope to perform assessment and plan treatment for patients and in doing so I should obtain the knowledge and skill set to complete this task.
