Finally, I would like to identify benefits and strategies to mitigate the challenges of interdisciplinary collaboration to plan care within the context of a remote team.
Collaboration among the different care providers and the patient’s mother has significant impacts on her health outcome. It enhances convenience, saves on cost, and prompts a response from connected healthcare providers. Different care providers can discuss the best ways to manage the patient and conduct follow-up remotely without the patient coming to the facility. The various care providers involved can also discuss any challenges that may hinder the process of monitoring the patients and come up with strategies on how they will be handled to ensure the efficiency of the entire process. Various strategies can be developed to address challenges facing the collaboration process. These include proper education of all the stakeholders involved on how best they can run the process. Education of the care providers, such as the patient’s mother, is essential as she will make any necessary inquiry depending on what she notices. Technological connectivity is also crucial between the care providers and the patient. They should be adequately trained on how to use them to ensure easy and effective communication (Swar et al., 2019). Addressing the challenges will make the process more efficient, thus enhancing patient health outcomes.


 

 

In conclusion, evidence-based practice models are useful in healthcare provision as they offer frameworks to apply EBP across various settings. EBP models help the baccalaureate nurse develop tailored plans of care by incorporating the most current and relevant research when making vital decisions about care. Besides, having the most current evidence and promoting the collaboration of an interdisciplinary team who communicate effectively leads to positive patient outcomes.
