Using this model, outline the steps for a family-centered health promotion. Include strategies for communication

The transtheoretical model steps to a family-centered health promotion entail collecting
information and conducting health needs assessments. In this case, the initial step will entail
requesting the family members to seek the identified screenings due to the relevant risk factors.
The second aspect will involve communicating information regarding the consequences of the
health issues identified for each family member in a manner that enable them understand the
perceived severity based on the risky behaviors (Braveman & Gottlieb, 2016, p 22). In this case,
the health promotion communication strategy will entail face to face meetings and distribution of
health posters that contain information regarding the causes, symptoms, and treatment options of
the identified diseases. The next step will entail preparing the family to take action and removing
all the barriers. For instance, i will request the father to have a flexible work schedule that can

afford him enough time to engage in physical exercise (Kumar, 2017, p 2). I will prepare a
nutritional and diet plan for the family and request the members to minimize their intake of snack
foods. The next step will entail preparing a strategy to maintain the new lifestyle and healthy
eating habits for the long-term by highlighting the attained short-term benefits like reduction of
weight and good sleep and waking patterns. In the end, I will eliminate the desire to return to the
previous negative behavior by assisting the members attain self-efficacy and personal
management skills (Kumar, 2017, p 3).
