Introduction This was a complete psychiatric evaluation performed on a 27-year-old man who served in the military after graduatingfrom high school. This Client's symptom is a Post-Traumatic Stress Disorder (PTSD) diagnosis. PTSD is a disabling psychiatric disease resulting from exposure to an actual or threatened injury, death, or sexual assault. PTSD can be linked to both functional and cognitive issues. Early detection and intervention are critical for effective treatment and minimization of any long-term consequences of PTSD. ( Mann, S. K.& Marwaha, R.2022). A wide range of symptoms characterizes PTSD. The Client's warzone experience exposed him to environmental stressors. In addition, after spending eight years in the military, the Client reported having multiple occurrences of hallucinations last year. This study aims to detail this Client's psychiatric examination to raise awareness of the signs and symptoms of PTSD. History of Present Illness: The major complaint of this Client is poor memories which can be directly traced to the period when he served in the military, according to his recollections. The Client's memories of wartime or warzone, no doubt, have caused irreparable damage to his social

Consequently, he has been unable to live and maintain a healthy social life—no wonder this Client often chooses to stay away from crowded events whenever possible. Additionally, the warzone experience and its memories have significantly affected his quality of sleep to the extent that he rarely sleeps through the night due to recurring nightmares. Even though this Client has reported several incidents of hallucinations, his cognitive ability and judgment seem intact. Past Psychiatric History: GENERAL STATEMENT: No history. CAREGIVERS: His fiancé is his primary caretaker because he lives with her. HOSPITALIZATIONS: No history MEDICATION TRIALS: No mention of current medication PSYCHOTHERAPY OR PREVIOUS PSYCHIATRIC DIAGNOSIS: Data not found. Substance Current Use and History:The Client denied alcoholism, Smoking, and use of illicit drugs. Family Psychiatric/ Substance Use History:The Client stated that his father is still a "sloppy alcoholic" but has avoided such addictions due to his condition. His father is similarly afflicted with hypertension, diabetes, and liver disease; his paternal grandpa suffered from depression, and he has not revealed his depression condition to anybody but the Client because of their common Combat experience. Mother is in good health and has siblings (one younger and one older sister)
