Referral Source: The patient has posited that she was referred to the clinic by the primary care provider

Chief Complaint: “I am always faced with an extensive feeling of worry, fear, and anxiety to the extent that my daily activities are affected”

History of Present Illness: The patient is a 32-year-old woman of African American descent. The patient has had issues of being worried and fearful to the extent that there is a challenge of being able to have a healthy living. The patient has had a healthy life but was diagnosed with having mental health challenges within the last year. The patient has had the challenge of interacting with others. The patient is not married and does not have children but is with a sexual partner. He is living with his parents at the moment. He has two siblings who are both healthy. His father and mother are in their 50s and they both have hypertension. She has denied having any issue with the use of the substance and has not made use of any issue of alcohol. She denied having any issues with allergic reactions.

Stressors

The patient is exposed to issues of concern as there is a lack of any factor that is increasing the cases of having fear or any worry.

Past History

The patient has not had any past health medical concerns. When she was 7 years old she was diagnosed with the issue of chickenpox. The patient has not had any challenges and that one is expected to have failed to use any issue drugs or issues of use of any drugs. The patient denied having any issues with hospitalization in the past….
