Throughout this week, I noted that it is crucial to plan the activities that one intends to
accomplish. After working with the preceptor, I developed an individual success plan that will
guide me in accomplishing the weekly assignment. Additionally, my preceptor helped me
perform the need assessment for my organization where I was doing my practicum. At the end of
the discussion, we had come up with five topics relevant to the practicum site, and they can be
essential for my change project. These topics include; catheter-associated urinary tract infections,
burnouts among nurses, medication errors, pressure ulcers among elderly patients, and
prevention of injuries. Furthermore, my preceptor taught me more about new practice approaches
that are vital in improving patient outcomes and providing quality care.
New practice approaches generally entail searching for the new solution to the problems
that are encountered in nursing practice. Statistically, it has been identified that the nursing
practice field faces a lot of issues when providing care. Therefore, it is crucial to search for more
information. The Discovery of new practice approaches in nursing will improve optimal patient
outcomes and contribute significantly to professional development (Everett & Davidson 2019).
I was able to recognize my strengths in the practicum area. Firstly, I was very concerned
about providing quality care to my best as a professional nurse and serving all patients. Also, I
was very much interested in identifying the problems or issues that affect my organization to
search for new solutions to solve such issues. Adopting evidence-based practice in nursing is
one of the additional resources I will recommend in my practicum site. Studies have shown that
evidence-based practices are the most effective way of solving issues in nursing practice. In turn,
they will positively influence the optimal patient care outcome (Spooner et al., 2018).
I utilized the Grand Canyon University library database and partly the google scholars’
articles to meet my course objective. This is because the GCN library scholarly database is rich

with many quality research articles relevant to my capstone project change proposal (Morelli,
2021). Additionally, most of these GCU scholarly articles are peer-reviewed; hence they are
essential in building an evidence-based practice in the nursing profession.
In conclusion, it is vital to have an individual success plan when carrying out any
activity. This is because it guides one in completing various tasks successfully. Also, it is crucial
to work in collaboration with the preceptor and course faculty when doing fieldwork at the
practicum site. This is because they have experience handling the change project proposal and
identifying the essential new practice approaches in influencing optimal outcomes.
